In early 2004, Anglo American Platinum division was faced with numerous project challenges including stage gate reviews during feasibility phase, awarding contracts to competent construction companies, managing contractors and an increasing number of safety related incidents on site.
It became clear from various audits and assessments conducted internally and externally that the management team was not aware of their exposure to risk and the subsequent consequences.
